Forms Quiz Add-On
The Forms Quiz Add-On allows you to quickly and easily create fun and interactive online quizzes which are automatically graded/scored after the submission of the form. When activated the Quiz Add-On adds a new Quiz Field which allows you to configure your quiz questions and answers; questions can be created as multiple choice (Radio Button), Drop Down or Checkbox Fields.
A Quiz can be configured to be graded either on a simple Pass/Fail scale or using a more complex letter grade system which can be customized as needed. You may choose to provide participants with instant quiz scores and answer explanations or not show results immediately. You have full control over what is presented to the participants when the quiz is completed.
The Quiz Add-On enables you to add as many Quiz Fields as you like to ANY form you choose! Learn how to create a Form.
Activate Quiz Add-On
Log into your site Admin Panel, navigate to the main menu on the left APP -> Forms then click on the Activate link.
Setup Your Quiz Fields
After the Quiz Add-On has been activated a Quiz Field will be displayed in the form editor Advanced Fields toolbox, this will enable you to add quiz questions to any form.
To get started creating your quiz navigate to the main menu on the left Forms. You may choose to either create a New Form or edit an existing form.
Add quiz questions
In the form editor click on the Advanced Fields toolbox to expand it and display additional form fields.
Click on the Quiz Field to add a question to your form.
You may now edit the Quiz Field to configure the available options.
To configure your quiz question click on the Quiz Field to expand it and display configurable options. Enter the question you would like to ask participants in the Quiz Question box/field. You may also enter a description (optional) in the Description box/field (this can be used to display additional instructions to participants and provide some direction on how the field should be filled out or selected.)
Select the Quiz Field Type you’d like to use for your quiz question, available options are Radio Button, Drop Down or Checkbox. You may choose radio buttons and drop downs if your question only has one correct answer or use checkboxes if your question has multiple correct answers.
Enter the answers for the quiz question.
- You can rearrange the order of how the answers are displayed to participants by using the up/down arrows to the left of the answer by dragging it either up or down.
- You can mark an answer as correct by using/clicking the Checkmark icon to the right of the answer.
- You can add additional answers or remove any answer by using the plus (+) or minus (-) options.
- You can enable weighted scores by clicking the Weighted Scores checkbox at the top of the answers. This will enable complex scoring systems in which each answer is awarded a different score. Weighted scores are awarded regardless of whether the response is correct or incorrect so be sure to allocate higher scores to correct answers. If this setting is disabled then the response will be awarded a score of 1 if correct and 0 if incorrect.
Check the Randomize order of choices checkbox to randomize the order in which the answers are displayed to participants. This setting affects only the quiz front-end. It will not affect the order of the results.
You may also check the Enable answer explanation option to display an explanation of the answer along with the quiz results.
To add additional questions to your quiz repeat the steps outlined under Add quiz questions
Click the Update Form button.
Setup Your Quiz Grading
After updating/saving any form with Quiz Fields you will need to configure the type of grading you would like to implement for your quiz.
Navigate to the Forms top menu/toolbar Form Settings -> Quiz.
Shuffle quiz fields checkbox to display the quiz questions in a random order. This doesn’t affect the position of the other questions on the form
Check the Instant feedback checkbox to display the correct answers plus explanations immediately after an answer has been selected. Please note that once an answer has been selected it can’t be changed unless the form is reloaded. This setting only applies to radio button quiz fields and it is intended for training applications and trivial quizzes. It should not be considered a secure option for testing.
Selecting the Pass/Fail grading option allows you to enter a percentage which must be obtained in order for a participant to “Pass” the quiz. Enter the Pass Percentage and customize the confirmation messages which will be displayed if the participant passes or fails the quiz.
The Pass/Fail Confirmation message will automatically be displayed after the standard Form Confirmation message when the form is submitted. The confirmation messages also support Forms merge tags.
Selecting the Letter grading option allows you to configure more advanced grading which can be customized to suit your needs. You may customize the Label and the Percentage which must be obtained in order to receive that grade.
You can also customize the quiz confirmation message which will is displayed automatically AFTER the standard Form Confirmation message when the form is submitted.
Finally click the Update Settings button.
Access Quiz Results
To access your quiz results navigate to Forms top menu/toolbar and click on Quiz Results. If you have multiple Forms Add-Ons activated which support results you may need to rollover/mouse over the Results menu item and then select Quiz Results.
On the Quiz Results page you will be able to view aggregate results of your quiz and filter this data based on date and field selection.
Display Your Quiz
Your newly created quiz can be displayed on any page or post simply by embedding the form which holds your quiz. Learn how to embed a form in page/post.
Quiz Merge Tags (Advance)
The Quiz Add-On introduces some additional new Forms Merge Tags which can be used anywhere within Forms where merge tags are supported. Merge Tags are mainly used in the Form Confirmation message and Notification messages to output the results of a Quiz within the content of the message.
{all_quiz_results}
Displays the results of all Quiz Fields that exist in the form.
{quiz_score}
Displays the number of correct Quiz answers the user submitted.
{quiz_percent}
Displays the percentage of correct Quiz answers the user submitted.
{quiz_grade}
Displays the Grade the user achieved based on Letter Grading configured in the Quiz Settings.
{quiz_passfail}
Displays either Passed or Failed based on the Pass/Fail settings configured in the Quiz Settings.
